Nous rapportons le cas d’une jeune femme de 24 ans qui a reçu, dans le cadre d’une analgésie péridurale pour le travail et l’accouchement, une injection accidentelle péridurale d’un mélangé de bupivacaïne et de fentanyl préparé à partir d’un flacon de sérum salé contenant 20UI de Syntocinon. L'injection a immédiatement provoqué des lombalgies très intenses, associées à des nausées et des vomissements. Les symptômes ont disparu quelques minutes après l'administration de 2 ml de lidocaïne sans adrénaline et de 6 ml d’une solution analgésique contenant de la bupivacaïne hyperbare et du fentanyl. Aucun déficit neurologique ni douleur persistante n’ont été observés durant son hospitalisation ni pendant les trois mois de surveillance à domicile après l'incident. L’injection intrathécale d’ocytocine dans le traitement palliatif des douleurs de neuropathies chroniques a été rapportée dans la littérature. Toutefois, il n’a pas été relevé de complications neurologiques après une telle injection contrairement au cas rapporté ici. La prévention reste essentielle pour éviter de tels accidents. Nous recommandons les mesures suivantes : élaborer des protocoles clairs pour l’analgésie péridurale dans notre centre de maternité, afin d'assurer une procédure uniforme et sécurisée. Il est également crucial de développer des protocoles de communication efficaces entre les membres du personnel médical et paramédical pour minimiser les risques d’erreurs. Encourager et instaurer une culture de discussion collégiale permet de détecter et d’apprendre des erreurs médicales, contribuant ainsi à leur prévention future. Enfin, il est important de mettre en place des mécanismes de déclaration immédiate des erreurs aux autorités compétentes pour une gestion rapide et une amélioration continue des pratiques.
