Abstract Résumé Introduction : L’endométriose est une maladie gynécologique chronique, souvent responsable de douleurs pelviennes et d’infertilité. Sa prise en charge reste complexe, surtout dans les formes profondes. Observation : Nous rapportons le cas d’une patiente de 40 ans, G2P1A1, suivie pour infertilité secondaire et douleurs pelviennes chroniques. Elle a bénéficié d’une kystectomie par laparotomie suivie d’un traitement médical par Decapeptyl. Deux ans plus tard, elle présente une récidive avec découverte d’endométriose profonde, confirmée par IRM. Discussion : Ce cas illustre les difficultés diagnostiques et thérapeutiques de l’endométriose profonde, dont la récidive est fréquente malgré un traitement combiné chirurgical et médical. La prise en charge de l’infertilité dans ce contexte soulève la question du recours à la chirurgie de la fertilité ou à la procréation médicalement assistée. Conclusion : L’endométriose profonde reste un défi thérapeutique nécessitant une approche individualisée et multidisciplinaire.
